·     Explain how your organization provides programming to youth.

·     Explain the specific program and the number of youth that will be served.

·     Explain the intended outcomes of this program.

·     Name Rotary members currently involved in your organization & their role (if any).

·     If Rotary, the organization, can be involved in this specific program, please identify how we can participate.

·     Explain how Rotary will be recognized for this grant.

A requirement of this grant is to submit a one page written report at the conclusion of the program.  The final report deadline is March 31, 2013.  If a report is not submitted, you will be eliminated from consideration in future years.
